Thank You Page is Blank

Good Afternoon,

I have created my 3rd form with your product, which I love by the way.  This is by far the most complex one we have done.  The problem is when we click submit instead of the getting the Thank You message we get a blank white screen.  People are thinking their order didn't go through.  I tried clearing the cache. My form is live so I created a clone for testing & have same problems.

Could you please remove the Upcoming theme and then re-apply the Upcoming theme once again to the form. Once the theme is applied, apply the styling once again.
