I made a form and when I add the link and try to open it, it asks for log-in.

I made a form and when I add the link and try to open it this pops up but my hair one comes out so I don’t know  what I’m doing wrong?

The link you used is for assigning JotForm users who have a JotForm account to your form. That's why you get that screen.
