how can i get a submission that shows all info on original form

i'm trying to create an registration form with an indemnity

i inserted my indemnity paragraph as text

when I recieve the submisssion from the client the indemnity text doesn't show on the download so the form is basically useless because all i have is contact info and a signature

how do get a version that shows all my text included in the final submission so i have the signed indemnity on file

is this possible???

i tried it again i sent the client a link to the form 

this time the client was able to complete the form but i'm still getting the same problem none of my text is showing in the submission

You can see from here that your submissions are working fine.

After downloading a single submission file from the shown option you can see your form along with the data in pdf.
