We have six child development centers

Is it possible:

1) if they select one center - response will go to a specific email address (6 different center which mean six email address specific to that center one each)

2) we would like our parents to complete through their mobile phone.  do they need to register or can we send the form directly for them to complete or will they need to join Jotform in order to complete.

We are thinking about getting the Silver package.

Yes, it is possible to send emails to a specific email address of a center if it is selected in the form. First, you'll have to set up Notification emails for all 6 recipients.

Then you'll have to create 6 conditions that will send out emails when a certain option is selected in the form.
