I have tried to upload a new image on your cloned form, and I was able to do it without any problems.
I have cleared your form cache, which should resolve the issue. Can you please try to empty your browser's cache, and try again? If that does not work, can you please try to use a different browser?
If the problem still persists, let us know, and we will investigate further.
