PDF file modifications

Hi,

I have a couple of questions.

1. How can I edit the name of the PDF file that will be emailed to the recipients? I want it to be dynamic and use one of the filled in questions from the form. Is that possible? Can it be something like: JotForm X - {CompanyName} for example?

2. Can we have an extra page in the PDF that is created? As in: I want to have a nice first page with some information and a photo that can be uploaded. Is something like that possible? 

3. The outline of some 'questions' in the PDF form are a bit off. Like for example, some questions are like this:

'What is y
our name?'

How can I fix this? I added a form so that you can see what I mean.

1) You are able to update the PDF name from the Customize tab:

2) To add a new page, you need to add a Page break at the top of the first page:

Then you can add anything you need to the new page, just make sure you add it above the newly added Page break: 

3) To resolve this problem, you can align the question above the user's answer. This option is found when you click on the question settings:
