I have lost previous reports

I received orders, (submissions) in July and early August.  I no longer see them on my report.  Can you help?

It looks you were importing a PDF fillable form, but as you can see below, no form is uploaded/imported:

Upon checking your Account history log, I could not find any submissions or emails sent for this form.

Hence, please confirm which form are you referring to for which you have received submissions in July and August.
