I copy and pasted the code I was given to distribute my form and I get a "page not found" error message.

You should create a JotForm account first before you proceed any further in creating your forms. Use this link to create an account http://www.jotform.com/signup . Use the same email that you use here.

You have not provided us the URL of your website where you might have embedded the form. Please share to us the URL of your website where the form was intended to be used. We will check further.
