I have gone through your website and it is amazing. I think you can help me to do something what I am looking for. Can you please create a form for me if i guide you what i really looking for.

Thank you so much for your prompt response. And sorry for the delay.
We would like to start a monthly mess service to customers. We would like to onboard the customer through online with basic details . I think jotform can support me.
first of all we want to be the page look like a website. Then our logo and name with tag line
then we need the following information to be show there.
about us (few text about our services)
menu (3 sliding pictures , if possible)
delivery location ( show in a drop down box)
Start my mess ( two options [one time - lunch /dinner] [Two time - lunch and dinner]
need to collect the following information from customers who selected two options
Name, Mobile number, what’s app number if different than mobile number, room address, office address, type of rice( matta/baric), delivery location options ( days from Sunday to Saturday with delivery location option for each days [room or office], date when they want to start the mess (a note that mess will be delivered for 30 days form the starting date)
if they select one time (lunch/dinner ) then we will collect all details same as above except few change like , Selection option (lunch or dinner) ,instead of room and office address we will make it as food delivery address, and other details will remain same.
Refer and earn(reference name, reference number)
contact Us( will show 3 mobile number)

this is all about the form. And our main intention is to collect all details from customers through online during the registration and need to get a full report in excel format as well as an e-mail confirmation to us for each register.

Hello Lijo - Currently, we don't offer form customization service but we can guide you while you build your form, and provide suggestions on how to achieve those requirements.
I have answered each individually:

If you have an image file on our end, you can use the IMAGE field and add it on top of your form.

These can be added using the basic elements/fields found on the left side of the form builder.

After building the form, you can then share it to your customers via email, then for each submission received, you'll be receiving an email notification containing the data by default. This will be sent to the email associated with your account, but you can change, in case necessary, by following this guide on How-to-Change-the-Email-Address-Used-for-Notification-Email.
And lastly, you can download the submission to an Excel format. Here's a guide for that: How-to-Export-Form-Data-to-Excel
