Difficulties setting up the appointment field

I am really struggling with using this feature.  When I start to put in the parameters, it defaults to today's date.  Instead, I would like it set up for Friday, August 21.  I need 15 minute appointments from 8 am - 3:30 pm and 2 appointment availabilities per time slot.  Please help.
