    This is what I get:

I have cleared browsing data and cookies many times as was suggested in other posts.  I tried renaming the spreadsheet.
I have deleted all files in my Google Drive, closed my browser (Chrome) and removed integration numerous times.  I also tried it in Explorer.  I hope you can help.  Thank you very much.

I'm not able to replicate that to my end. Please try to first remove the broken integration and integrate it again. Clear your forms cache as well: How to Clear your Form Cache.

Let us know for the results.

Since none of the clearing of browser dataworks, I highly suggest that you create a new account on google and re-integrate using the new account. And if problem persists, kindy let us know then we'll investigate further or escalate the issue to our higher level of support.

    Thank you for using JotForm

Your spreadsheet can be found here:

I'm using Chrome.  Could that be a problem?

You've reached the spreadsheet maximum column limits that's why integration can't provide you a working link.

Google spreadsheet maximum column limit is 256 per sheet. Looking at your form, you have a matrix element which has 35 rows with 10 columns. If we calculate that, that's equivalent to 350 columns or more. Note that matrix element creates a separate column for each cell that's why I multiplied 35 by 10. You may try our reports feature instead like Grid Listing and HTML Table Listing. Here are the guides:
