I want to add initials on each page, signature and date on the last page of a 7-10 page existing PDF consent form. Is this possible

You can add an initials field in the form and a date field and move them to show at the bottom of the page.

If you want to add them to multiple pages, you need to add multiple hidden fields.

Since the user will most likely fill the initials field you can copy the initials field contents to the other hidden fields.
