Form send button not working

The send button on my form not woring

Is this issue with respect to all the forms or with any particular form? Is the form embedded on any website?

I isolated it to the appointment widget will accept a selection but the form submit button does not work only while card form layout is selected
...

I was testing a demo cards form of mine but could not replicate any issue with it.

May we know the form in which you are experiencing the issue?
