How to include the appointment date in autoresponse body?

Hi-- sorry again- i figured out how to send out the autoresponder to the sender's email. but the question I have now is, we have a calendar feature where they make their appointment (day and time) 

how do i allow that piece of information to be included in a sentence format. 

for example- i'd like to to say, "Your appointment is scheduled for (appontment)" 

Please check my screencast below on how you can include it into your autoresponse email body.

The idea is just you click on the "Form Fields" button to select which field to insert in your email body.

why is that?? the only things that show in the FORM FIELDS drop down for me
are SUBMISSION ID AND FORM TITLE.

Hello Kathy - It seems you already figured it out:

It could be that the PHI is enabled for the Appointment field at the time you're creating the email, reason why it doesn't show up on the choices. The PHI should be disabled:

I just tested your form and I was able to get the appointment field date appear in my notification.

May I know if you have already tested it? Looking forward for your response.
