I have created a form with conditions, and it is not working properly

Hi, I have created a form with conditions under my account and I can't get past a specific page 4 unless I remove all conditions.  I have removed and redone them as well as cleared my cache.  I am not sure what the problem is but have spent several hours troubleshooting to no avail.
