My forms/account has been suspended due to phising

    my account has been suspended and myself and my customers cannot accuess my registration forms due to 'phishing'. I have no idea what this means and I need it rectified ASAP. They are getting 'page not found' and I cannot log into my account.  PLEASE help me with this issue immediately.

    In this case, the account has been suspended because some of your forms are requesting credit card information, and base in our terms of service it is considering as phishing.

    Our Terms:

    Phishing. You agree that Jotform Inc. may terminate your Jotform Service immediately if a form’s purpose is found to be deceptively obtaining, for example, sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    In order to reactivate the account we need:

    1. The URL where your form is used.

    2. Your website or proof of affiliation to a legitimate institution.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Send these proofs to with Thread #2530209 as your subject. We will let you know about the status of your account after a thorough review of your proofs.
