Why has my account been suspended?

    Hi Jotform Support team,

    I am new to Jotform and I am trying to see what can I achieve with Jotform services. I was trying to create 2 forms, which one of them is a register form. My objective is when a customer register for the first time, they need to enter a password in order to use the second form. When I was inserting the password fields, my account was disabled for phishing purposes. The password that the user need to fill in is the password that they will need to enter on the second form in order to watch the other fields.

    Can my accoutn be enabled again please?

    Kindly be informed that your account has been suspended due to phishing activity within your form in which you ask users for "Login Credentials as shown below:

    Furthermore, online Form Builders, including Jotform, are sometimes being used for identity theft, and we go to great lengths to prevent this. That's why collecting these types of data is considered as phishing activity, as stated in our Terms of Use.

    I've successfully removed the offending fields and reactivated your account. You may login now with no issues at all.
