Why has my account been suspended?

    I have requested a password reset and not receiving it.  I created an account and received an initial email saying that my user name is wcedp.ad and I am not getting in either way.

    Upon checking, I've found that you're offering a service using an account registered with a free domain email. Therefore, in order to reactivate your suspended account, we will have to delete the offending form Business Loan Application Form.  Or, kindly provide us with the following information:

    1. The URL where your form is used.

    2. Your website or proof of affiliation to a legitimate institution.

    We also need you to change your registered email to a corporate one to avoid getting suspended again. Send these proofs to reviewers@jotform.com with this thread number, which is 2532715, as your subject.

    We will let you know about the status of your account after a thorough review of your proofs.
