Form is not submitting

    We had a visitor contact us that they had submitted a form and had a short flash of a red error message and then they could not see a submit button any longer. It seems that we did get a copy of that form but no notification and they did not get a confirmation.

    We tried to submit it ourselves and it went normally but it is not showing up in our submissions and again as a sender or the email manager, no notification is received. We do not embed it in the page. They fill it out on the site here.

    I did clear our form cache. Can you please advise on how we can fix it or what might be wrong. Do you need a specific php level on the server?

    I cloned your form and test submit it and it's working as expected. 

    To clarify, they fill out the form using the stand-alone form, right? Could you please let us know if we can send a test submission to the form to test it from our end?

    Thank you. We link to the form on this page: https://www.blueotter.com/forms.php It is the form "Submit Float Plan" and it links to https://form.jotform.com/ggibbs/blue-otter-outfitters-float-plan--e

    Yes, please send a test submission. I can not connect with the account holder until tomorrow AM but will let them know to confirm with me if they get a submission from you.
