My account was suspended

The forms I use are to collect information from patients who I see for counseling. All form links will be sent via a secure encrypted message.

Hi Keyah,

Thanks for providing us the info,

I have checked your account and it's marked as suspended because of the form ID(202024292991049) requiring SSN/Credit Card Info, which is against our terms and condition. 

In order to reactivate your account, we will have to delete the offending forms.

Will you only be deleting that one form from the entire set of forms? If so, that is fine. I would like my account reactivated.
