I have created a product order form but I do not want to use the credit card part

I have created an order form but I do not want to use the credit card part of the form.  I can't seem to remove it.  I started with a templet for product orders, I made the adjustments but I just want my customers to submit it and I can invoice them.

How do I remove this from my form?
