I'm currently on a trial.

This template has the theme I want... but how do I use this theme in other new forms?

The templates cannot be used as themes, for example, if you use a template, this will create a new form.
Now, the one you shared has the "Upcoming" theme which can be added from the Form Builder, here are the steps:
1. Edit your form and click on the designer button:
2. Go to the "Themes" tab and use the "Upcoming" theme:

One more question: I made some customizations to the template. How do I save it for future use?
I went to https://www.jotform.com/mythemes -> "Create New Theme" but it then says "This form is not compatible with this designer."

Hello!

You will see this error for the forms using the "Upcoming" theme as it is not yet available to be created as a theme. For many other themes, including our Default theme, you won't see this message.
However, you have the option to save the same as a template. These form templates can then easily be cloned to create a new form. But do note that this action is public. Your form template will be available to be cloned by the whole JotForm community.

BTW, we also have form cloning feature. Form cloning allows making a duplicate copy of the form (without its submissions). As you need to create a template for personal use only, you can keep using this from and when the need arises, you can clone your own form to make a new copy of it.
