Issues when injecting HTML code from JotForm into SquareSpace website

    Hi,


    I just created a fillable PDF form on JotForm and I wanted to embed it into my website that I made on SquareSpace using the HTML code that you guys generated. However, after injecting the code, it's not formatting properly, certain elements of the form are missing/misplaced, etc. I've attached a screenshots below of the PDF as it properly appears as on JotForm, you can compare it to how it is actually appearing on my website with the link below. Please advise.

    Checking your website, I've made sure the form is properly embedded as shown below.

    As per your screenshot, kindly note that is a PDF form, not the original form shown above. This kind of PDF forms can't be embedded. However, it can be downloaded as shown below.
