I want to sign up for JotForm. But can't until I have absolute clarity on the following:
a). I want to be able create my Forms using JOTFORM but bind the fields to my SQL Database, so that when it's submitted, I can view the records from my third party app (fetching the submitted records from my db).
b). Is it possible to submit the form as an attachment in extensions such as Excel?

Adding form entries to your MySQL Database is possible. We have a guide for that: How to Insert/Update Submissions to Your MySQL Database Using PHP.

May I know if you are having problem with your database getting the data of your form?
Have you tried using Webhooks? This feature would replace Send Post Data + Custom URL Thank You Page workaround in order to help you transfer forms data to your own database using your own script codes.
