Is there a way to send a reminder based on an appointment day/time?

We do have an option for you to send an autoresponder email at a later data which can serve as a reminder email.

However, the setting can only read Date fields and not an Appointment field.

As a workaround, you can instead create a condition that will copy the Appointment field's value into a Date field.

Then use that Date's value on your autoresponder email's Send Emails Later section.

Also, please note that the setting can only select Dates but not time, as such, if you want a reminder email, you may want to send the email a day before their scheduled appointment. If you want this process, you can check out this similar ticket:
