I created this form (see the URL below...View Rates tab) that is supposed to be a checkbox, not three checkboxes with the text beside them. It's like the Agree to TOS and PP you see when someones signs up to a service. The check box is a required field, and I want the visitors to check the box to assure that they are potential clients. The issue is that I have already modified that form several times to keep only the single check box, but every time I check, I see the three checkboxes with the texts Option 1, Option 2, and Option 3.
What do you think is the problem here? Please help. Thanks.

To remove a checkbox:
1. Click on the checkbox field
2. Click the Options button
3. Remove the lines Option 2 and Option 3. Click OK
Remember that each line corresponds to a checkbox option, so if you want your options to be Yes and No, you would have to enter those words in each line, like this:
