    My account has been suspended .. Please help me!

    Hi Athanasios, 

    Your form here, is requesting passwords which is forbidden by our terms & conditions. Specifically, "Collecting Sensitive Information. You may not use the JotForm Service to collect certain types of sensitive information, including but not limited to ... any type of login credentials."

    If we have your permission we can remove the non-compliant fields and then re-activate your account, let us know if you would like us to proceed.

    Yes you can remove non-compliant fields.

    Hi Athanasios,

    We would like to inform you that the offending fields have been deleted from your form. Your account has been reactivated now. You should be able to login to your account now.
