I cleared the integration, logged out, cleared my cache and cookies, and started the integration process again. I authenticate, accept, then blank screen.

I can't seem to reproduce the problem. Would you please try it with different browsers like Chrome or Firefox? Also, try to login your gmail account with google first before integrating your form, this might help.

Now when I try in Chrome or Explorer, the same thing happens: It authenticates and initializes the spreadsheet, but the url has an error at the end of the address:

I think I have explained this to you 4 days ago. Anyway, incase you missed it, here's the reason why you're not able to get a working spreadsheet URL.
You've reached the spreadsheet maximum column limits that's why integration can't provide you a working link.
Google spreadsheet maximum column limit is 256 per sheet. Looking at your form, you have a matrix element which has 25 rows with 10 columns and other extra fields. If we calculate that, that's equivalent to more than 256 columns. Note that matrix element creates a separate column for each cell that's why I multiplied 25 by 10. You may try our reports feature instead like Grid Listing and HTML Table Listing. Here are the guides:

Yes, you did help me a few days ago. So I reduced the number of rows to 25 from 32, I believe. 25 x 10 is 250, and you said the limit is 256. But it's still not working.

Yes, it's 250 but like I've said, you have other fields not only the matrix field so that's still counted. Make it 20 rows, that should solved this.
