Existing jot form

Hi someone created a jot form for me and I've been using it but I received a notice saying I've reached my capacity.  I signed up and paid for an account but I don't know how to link that form to my account because it goes to my other email address. I'd like to keep that form because it is already posted to multiple locations.  Are you able to help?  I have no idea how to create a jot form, or what it is because it was done for me but I really need it to work.
