    how do you get rid of, 'Script embedded in HTML' on the web page?

    Hi,

    That's not from JotForm. I think that's added on your page to filter some scripts or something. I have no clue how to removed it since you are running with PHP. However, if you open your "Activities.php" you may be able to find it there.

    Here's my findings, the JotForm code is pretty far from the issue where it occurs. Kindly review it on your script.
