The one form we have set up with Jotform has suddenly stopped working, and after logging into our account there is now is no record of it, or any of the entries received over previous year.
I can't now find the original form address, but it an embedded form at:
http://www.blackswan.org.uk [backslash] marketing
After completing the form you now receive the error message:
Details: Form did not pass status check
Please can you advise how we solver the error, resinstate this form and get back all the previous data. It is a critical information gathering tool for our arts charity.
