How can I unlock access to my form and start over?

I am trying to my personal Artist profile and Submission Form on JotForm using my artist name (Simo Neri) and email address (simo@simoneri.com), but my passwords are rejected as invalid. 

It might be because the computer I'm using defaults to another of my email addresses (simoneri.shop@gmail.com) and names (Simonetta Neri). 

What can I do to unlock the form so that I can enter my info by the submission date?

The form was password protected by the owner, unfortunately, you will need to contact him and request the password in order to submit the form.
