    I've managed to embed the form in my test site, but couldn't get iFrame to work using the copy and paste of your code.  I kept receiving T String and Parse errors and this is beyond my understanding.

    Here's the code I used which works - but the form is tiny and scrolls.  I'm trying to get it to appear embedded in my site in the correct size as designed.

    <?php
    require_once( "class2.php");
    require_once(HEADERF);

    echo "<IFRAME SRC='http://form.jotformeu.com/form/32225303119341'></IFRAME>";

    $ns -> tablerender($caption, $text);
    require_once(FOOTERF);
    ?>

    Thank you for your continued help - I really appreciate it.

    Hello again!

    I have managed to do it.  Here's the code I used in case it helps others.

    <?php
    require_once( "class2.php");
    require_once(HEADERF);

    echo "<IFRAME SRC='http://form.jotformeu.com/form/32225303119341' SCROLLING='no' WIDTH='800' HEIGHT='1200' FRAMEBORDER='no'></IFRAME>";

    $ns -> tablerender($caption, $text);
    require_once(FOOTERF);
    ?>
