I've recently created a form and tested it by sending a submission. On the spreadsheet that is created to show my submissions, there seems to be data missing from it.
The details are showing, including Name and address etc, however the answers to the questions are missing from the spreadsheet. I've also looked at the submissions through the JotForm website and it's exactly the same.
Could you offer me some guidance?

Is this form embedded to your website? Have you modified the Google Spreadsheet? Take note that we do not advice modifying any part of the spreadsheet, specially the field headers. Also, I've check your form submissions and all the data submitted are there.
Another option is to re-integrate the form to Google Spreadsheet. Please try that and let us know for the results.
