Help with the New Patient Face sheet

Just click on top for Patients, then click on Forms - New patient face sheet.

That is where is problem is wouldn't allow patient to submit the form. I have tried again.

Hi! The problem that you were facing occurred because many of the fields were set to the datatype Email Address in your Fill-in-the-Blanks Form fields. Providing a non-email value is an error for such fields.

I cloned your Form into my account and fixed this issue. This is the new Form under my account: https://form.jotform.com/202553188344961

Please free to test this Form by filling it and then submitting it (you can test it multiple times). If you find any issues, let us know.

Otherwise, I would suggest you to clone this Form into your own account by following these instructions and embed this newly cloned Form instead of your original Form on your website.

This is because fixing it on your original form would take much time as you have lot of problematic fields, also I noticed that on your original Form you only have test submissions, so it is safe to change the Form at this moment.
