After I upgraded to $9.95 I have notice that once my vendor form is submitted, the actual email itself does not show my email

Your form isn't going to show up in your email itself as it doesn't work that way. What you will receive in your email is a copy of the Data that is filled in when someone has submitted information through your form. When I checked your form I noticed that your Notification is empty except for the word " undefined " so you will need to fix that or delete the notification and make a new one to correct it.
