Create HIPAA compliant Consent forms

    I work in a pharmacy and I am needing to create an immunization consent form and wondered if I can do that with your platform? I will have several questions to ask patients on intake to verify that they are eligible to receive a vaccination and then I need them to sign or give consent to receive their vaccination. I also need to make sure that these forms can be emailed or we can be alerted when they are filled out and that the platform is completely HIPAA secure. Lastly can we print the forms out for record retention on site?

    Yes, you can create consent forms with JotForm and I suggest that you check the templates that you can find on our templates page.

    You may collect your respondents' signature electronically within the form using our signature field/widgets.

    You may also add the Term&Conditions widget where you may include what the agreement covers. It will have a checkbox that your respondents will check if they agree.

    The HIPAA Compliance feature of an account is available on a certain plan subscription level.

    Lastly can we print the forms out for record retention on site?

    The Submissions page, Inbox page, and PDF Editor page of your form all have a print submission option.

    I suggest that after you create your form, make test submissions, and visit these pages to test the Print option.
