I cannot embed my form and when a submission is made it comes up and says for disabled.
I checked the forums and most disabled forms are because of password fields. My form does not ask for a password. The form is a registration form to collect information from our customers about the classes in which they are registering their students.
I chose JotForm because you allow me to make a complicated form look simple using conditions, but this has been driving me crazy because I can't figure it out.

We apologize for this. I have now reinstated your account, so, you can now continue using our services.
Our anti-phishing system is a bit too strict. It autosuspended your account previously probably due to certain conditions (i.e. email, password combinations).
But I have now set everything properly so it should not happen again.
