I have my own smtp installed but still can not send any notifications and the form itself is NOT sending at all!!! What did you guys do or change about 2 weeks ago because before that everything worked perfect.
Can you please investigate my form and see why it is being blocked!!!

Try for yourself and see if it will let you send to this ONE email address; firstname.lastname@example.org
The "From Email Address" should be set to "email@example.com"

Good day! Is this the form in question, http://www.jotform.us/form/31168470562151 ? If yes, base on this form settings, you want the Sender (Your Email Field) to receive your Email Notification. This is somehow incorrect and/or the email settings using field tags is not supported anymore. If you want to send email to the sender after submission, use Auto Responder (Setting Up Form Autoresponder) instead.
Also, your SMTP Settings are also incorrect. The hostname should not be just "root". I would suggest checking and correct the details or setup SMTP using Mandrill. Follow this guide for this settings: Send Form Emails From Your Own Email Address: JotForm Now Support SMTP.
