The man who made our company website is no longer with us. He was the webmaster for the company and when he made the company website he listed himself as the primary contact on the jotform listed in the website.
He is no longer with us and I need to take his name off of the contact form but I cannot find the original jotform in a way that can be put on your platform for simple editing.
I've attached two links, one to the source code of the original jotform, and two the company page where the jotform is embedded that I need to make changes to. Please let me know if there is anyway to make this change without completely recreating the jotform.

If you have access to modifiy files in your website then you can clone the form and add the new one, it's pretty simple:
Take a look at this guide: http://www.jotform.com/help/42-How-to-clone-an-existing-form-from-a-URL it explains the simple process to clone a form
You can create a new form with your account and select "Import form" from a URL and then paste the url of the form "http://www.jotform.com/jsform/3084152229" and you will now have your jotform once you finished now you're gonna have to access your website files and edit the "enroll.html" file there is a code inside it wich contains the following:
you're gonna have to simply modify the number with your new form number, and you're done.
