    Three drop down fields are not saving

    In my form "Physician Preference Form", on page 2, there is a heading:"Red Flag 1 Intensive monitoring" with three (3) drop down fields under it. There three instances of this in the form (Red Flag 2 & 3) where the drop down fields do save, but the fields under the heading Red Flag 1 are not saving. When I enter the form link in the browser address field and go to the form, the drop downs are not under the heading "Red Flag 1 Intensive Monitoring".

    Please help

    Hello lbrmd,

    I cloned your form in my account. You have created three conditions where you check "If Red Flag #1 Is Equals To "....." Show ..."

    These conditions are actually the reason why the three drop down under "Red Flag #1" doesn't show up. It seems that "Increase UDS" & "Increase Pill Counts" text have been changed in the checkbox options so the conditions doesn't match anymore. (You must have changed the checkbox options after creating your conditions.) Your third conditions works fine and when you select the "Check in" from the "Red Flag #1" options, the drop down does show up. Please check this image:

    If you don't need these conditions, you can simply delete it and your all three drop down will show up. If you actually need to have these conditions, you need to update your conditions.
