My edited form still has the same submission response as the template

    When I view my form on the Jotform website, it looks fine. When I receive
    my notification email, or view the notification settings within Jotform,
    the questions are still the original templat'es that I used and changed.

    It sounds like you need to re-embed your form. Please refer to this guide if needed for reference on how to add a page to your website. If re-embedding your form does not work please send us the url for the webpage where your form is embedded so that we take a look at it and provide a better solution.
