How to access my form.

The form from my site is not showing all of the signatures and info that I required.

I created a liability and waiver from for my website and it is emailed to: twistedanchorrentals@gmail.com. The website is: www.twistedanchorrentals.com

I don't think I set up an account so I can't access the from. My other google gmail is: arousseau.morris@gmail.com so it may be listed under that.

Please let me know how to access my form and what to do about the missing info on the submission.

The link is to the form off my website. I can't send a screenshot due to privacy for my customer. Mainly the signatures didn't come through and the names and ages where jumbled.

The form is under the account am***11@yahoo.com. If you own this email but do not know your JotForm password, kindly request for a reset password.
