I created this quick form with several different required fields. Once the customer fills out the form, not all fields are being sent to my email. See attachment.

I am sorry for the trouble caused to you. I did check your form and found that you have 7 fields and all the data is being captured correctly in form's submission page. The issue seems to be with the notification email alert which has few other fields which is not available in your form.
The best way to solve this problem is to delete the existing email alert and add a new one. When you add a new email alert, it will always include all the questions of form. The following guide should help you:
