How to make form so when I send to excel, I can get right info to column

Hello,

Not sure how to ask this question. I'm trying to make a form for my field service guys so that when they apply a product they put on the form the product type and how much used in pounds. Problem is, I want to make a spreadsheet and have the accounting person be able to get the amount in lbs and charge accordingly to product chosen to the customer. But Im not sure what widget can do that to make it easy for them to filter out.
