We just updated our status with JoTform. However we lost the contact form on our page. Can you re-instate from your end?

Your account has been de-activated due to Overlimit of SSL submission.
But your forms are still intact.
I highly suggest that you upgrade your account to increase your Submission limit including the SSL submissions.

In addition to my response,
I manually adjusted your SSL submissions counts to 9 and 10 is the maximum on free plan (this is necessary to remove the Overlimit). I also activated your account.
Please be inform that we can only accomodate this manual adjustment at least this time only. Try not to use the SSL submissions if not necessary.
But if your requirement exceed the limits for the Free subscription, I highly recommend that you upgrade for paid plans. As I have mentioned it from my last response.
