    Hi  suport,

    I want to create a form for our website in jotform, Is it possible to crete conditional form like below: or see attached file. please confim me how to create these form.

    Hello Arvind,

    It's very much possible to create forms similar to your requirement. You can use conditional logic feature to create forms where you can show/hide form fields based on certain conditions.
