Fill in the Blanks>>Date Picker Fields: The Calendar does not pop out in the Fill in the blanks field

    Greetings. On the last page of this form, the date picker fields aren't working. There are four fields with dates only the first brings up a calendar. The rest do not, and you can only enter one digit into the date. I've tried clearing the cache. Thanks in advance
