    Hi,

    Good day! The best solution to fix this is to re-create your form notification. 

    First Step: Delete your current notification by following this guide: How to Delete Notifications/Autoresponder

    Second Step: Re-create your Form Notification by following this guide: Setting Up Email Notifications.

    We can also do this for you, just please let us know or give us authorization to do so.
