    Can I get more details on steps? Here is only step1 and step 2. I do not know how to do the step 1 or 2 at all.

    Your domain Registrar is GoDaddy, so you will need to add a DNS record through GoDaddy's site.

    Step 1. Add CNAME DNS record through GoDaddy.

    GoDaddy Adding or Editing CNAME Records

    I think that you will need to enter the following values.

    Enter an Alias Name: forms

    Points to Host Name: custom.jotform2.com.

    Note: you may contact GoDaddy Support, so they will confirm the settings and help you with this.

    Step 2. Use custom form URLs.

    Once the CNAME record is ready (it might take several hours or more to propagate over the DNS), you will be able to use custom form URLs.

    For example, the standard URL is:

    http://www.jotform.us/form/32388942901157

    And custom URL will be:

    http://forms.abcmanagementllc.com/form/32388942901157

    Standard embed code:

    <script type="text/javascript" src="http://form.jotform.us/jsform/32388942901157"></script>

    Embed code with custom URL:

    <script type="text/javascript" src="http://forms.abcmanagementllc.com/jsform/32388942901157"></script>

    under Cname
    quick add
    Host? Point to?

    Try the following.

    Host: forms

    Points to: custom.jotform2.com

    Then, we will need to wait for a DNS propagation time (usually up to 48 hours).

    Instead forms, Can I use applicationform, inspectionform, surveyform? add
    more than one hosts and all point to *custom.jotform2.com?*

    Yes, this is right, you can have as many aliases pointed to *custom.jotform2.com* as you want.

    So this will be:

    applicationform.abcmanagementllc.com

    inspectionform.abcmanagementllc.com

    surveyform.abcmanagementllc.com

    etc.

    You do not need to embed anything in JotForm. If you would like to embed the form to some page, you can use your custom name in the embed code.

    Grab embed code from JotForm, and change the form URL on it to your custom alias URL.

    Why do I point to custom.jotform2.com, not to exactly form?

    You point every form not a single one.

    It means, that you will get form URLs like:

    http://application.abcmanagementllc.com/form/32388942901157

    http://application.abcmanagementllc.com/form/22222222222222

    http://application.abcmanagementllc.com/form/33333333333333

    etc.

    So this will point each form to each alias.

    Where do I put the code, in the hyperlink?

    Mentioned embed code is for web page, for example, you can embed it to the:

    abcmanagementllc.com/form.html page.

    Direct form links do not have JS part (application.abcmanagementllc.com/jsform/):

    http://application.abcmanagementllc.com/form/32388942901157

    OK, I get this part worked.  Do I have to use the 4 digit bunbers or alias will work? How?

    What do you mean exactly with 4 digit bunbers?

    An alias should work with any of your forms as long as you changed http://form.jotform.com with your custom domain http://application.abcmanagementllc.com as explained by Mike.

    e.g.

    From:

    http://form.jotform.com/form/32388942901157

    To:

    http://application.abcmanagementllc.com/form/32388942901157
